Hey any of you that have ever filled out claims for AFLAC. We are hopefully going home today after Brennan's BT shunt. I've never filed a claim and was wondering if anyone could tell me the best way to get the forms filled out and get AFLAC to pay you all the benefits.

Do you have the Cardiologist fill out the Physician statement out once at the end of your stay and cover everything? What about multiple policy's? Do you have to fill out a form for each policy? I'm just a little confused....

When I filled our claims. I needed the surgical report, hospital bill and the claim form from Aflac, I had to have Dr. Kouretous (surgeon) sign the Aflac claim. It did take a while to do it via mail, so if your still at the hospital go the the cardiothorasic office and get the surgical report, then print the Aflac claim off and have them sign it before you leave if you can.

It is so much faster if you do it that way :) I would then fax it to our Aflac lady, she didn't get the 2nd claim I sent so I just sent it in the mail. Sorry it does take a while, but once Aflac gets it they are pretty fast to get you your money. GOOD LUCK!!! Glad your going home!!!
